Register Your Survey

The survey registration process varies depending on whether you are a current NC State administrator, faculty, staff or student, OR if you are not currently affiliated in any way with NC State.

For those currently affiliated with NC State: The Survey Registration Form External Link

If you are a current NC State administrator, faculty, staff, or student and planning on administering a survey targeting members of our campus community (i.e., students, alumni, faculty, staff, or administrators), as per Regulation 01.25.17 External Link you must first submit the online Survey Registration Form External LinkNote:  Please carefully review the Regulation and the related Standard Operating Procedures External Link document to determine whether or not your survey project falls under the purview of the Regulation, and to get a more detailed understanding of the process.

The Survey Registration form will ask for some basic information about your survey, such as an overview of the topics covered in it, who is included in the population, when you plan on administering it, and the IRB External Link (Institutional Review Board) status of your project.  See “Useful tips” below.

For those NOT currently affiliated with NC State: The External Survey Request Form External Link

If you are NOT a current NC State administrator, faculty, staff, or student, and want to target members of our campus community (i.e., students, alumni, faculty, staff, or administrators) to include in your survey population, as per Regulation 01.25.17 External Link you must submit the online External Survey Request Form External Link at least four weeks prior to when you hope to launch your survey.  Your proposal will be reviewed by Institutional Strategy and Analysis, and a determination made as to whether or not to grant access to your requested population.  Note:  Please carefully review the Regulation and the related Standard Operating Procedures External Link document get a more detail understanding of the process and the criteria used in the review.

The External Survey Request form will ask for some basic information about your survey, such as an overview of the topics covered in it, who is included in the population, when you plan on administering it, and the IRB External Link (Institutional Review Board) status of your project.  In addition, you will be asked to describe why it is important that NC State participate in your project, what, if any, benefits there might be for our participation, and who at NC State is familiar with your project and/or might be supportive of it.  See “Useful tips” below.

Useful tips:

  • It is helpful to see a copy of the registration and/or request form before completing it online.
  • You are strongly encouraged to consult the NC State Survey Activities Calendar to see if there are other surveys being administered during the same time you are planning on administering yours.
  • Prior to completing your form it is very helpful to first speak with a member of the ISA staff to discuss any questions or specific requests you might have.
  • As per Regulation 01.25.17 External Link, most projects will also necessitate researchers working with ISA to identify the population and select the sample for their survey. Before ISA can provide contact information for the selected sample researchers must submit the Sample Request and Confidentiality Agreement Form.
